Medicine Forms

 

Prescription Medicines

If your child has prescribed medication that you wish the school to administer, we need to have authorisation by your General Practitioner, Hospital Consultant or appropriate health professional. Without their signature, authorised staff cannot give any type of medicine to the students in school.

Please would you kindly ask your Doctor/Consultant to complete the “Consent form for the administration of prescription drugs” form and return it with the medicines prescribed to Student Services. You will need to have a new form completed if the type and dosage of medicine is changed. The medicines MUST also be provided in their original packaging (not broken down and placed in envelopes). All medicines will be returned to parents to arrange for safe disposal when no longer required.

Non-Prescription Medicines

Alternatively, we can hold non-prescribed medications (paracetamol, ibuprofen, antihistamines etc) in school, for students who may need medication for minor ailments.
Please complete the “Consent form for the administration of non-prescription drugs” form and return the form with the medication to Student Services.  Separate forms will need to be completed for each non-prescription medication we hold. The medicines MUST also be provided in their original packaging (not broken down and placed in envelopes). All medicines will be returned to parents to arrange for safe disposal when no longer required.
